Transitions at Claremont is a new, short-term rehabilitation unit for Claremont Nursing and Rehabilitation Center, the county home for Cumberland County, Pennsylvania.

The new, 39-bed unit was adapted into an existing shell space used for storage. The center provides a full physical therapy gym and occupational and speech therapy services with treatment options for orthopedic, cardiac, neurological, and pulmonary patients. Amenities include a country kitchen, dining area, living room, and spa.
